When facts change, the page must change

Content is not a one-time release. A page is reviewed, revised or withdrawn when evidence expires, authorization is revoked or sources conflict.

Report a correction

Include the public page, disputed text and a checkable source. Reports enter our review queue; submission does not immediately change the page.

Use 10–2,000 characters. Submit public facts and sources only. Do not include medical records, account credentials or other sensitive information.

Issues you can submit

Incorrect facts, expired sources, statements outside the authorized scope, missing key information, outdated versions or potential compliance risks.

Possible outcomes

After review, the original text may remain, receive clarification, be replaced by a new version or be withdrawn. New versions retain a revision date and version number.
